What to expect

Membership includes:

• Unlimited primary care visits (mobile + telehealth)

• Same-day or next-day appointments (when available)

• Direct access to your provider (phone, text, secure messaging)

• Preventive care and wellness visits

• Chronic disease management

• Medication management and refills

• Basic in-home/mobile procedures

• Care coordination and specialist referrals
